How to apply for SSDI in Rhode Island?

  • Medical Records: Detailed medical documentation, including diagnoses, treatment history, doctor’s notes, hospital records, lab results, and medications
  • Work History: Job titles, duties, dates of employment for the past 15 years
  • Income Information: W-2 forms or self-employment tax returns for recent years

C. Apply In Person

You can also apply at your local Social Security office:


  • Find your nearest office using the SSA office locator
  • Call ahead to confirm hours and whether an appointment is needed


4. Complete All Forms Accurately

During your application, you will need to:


  • Provide detailed descriptions of your medical conditions and symptoms
  • Explain how your condition limits your ability to perform work-related tasks
  • List all treatments and medications with no omissions
  • Report your work history thoroughly, including all physical or mental requirements of each job


Incomplete or inconsistent information can lead to denials or long delays.


5. Await a Decision

Once your application is submitted:


  • Your claim will be forwarded to the Rhode Island Disability Determination Services (DDS)
